The referral lifecycle

From submission to closure — one record, five stages.

    Sent

    Referring staff submits the structured form; a unique Referral ID is generated.

  2. 2

    Seen

    Receiving agency opens the referral; the referring staff member is notified.

  3. 3

    In Progress

    Receiving agency makes contact and begins client enrollment.

  4. 4

    Completed

    Service has been delivered — the referral loop is closed and logged.

  5. 5

    Unable to Serve

    Agency declined the referral with a documented reason; referring staff is notified.

Client identifiers are de-identified unless the client has signed a consent-to-share release. Data is encrypted in transit and at rest, role-based access keeps referrals scoped to the right agency, and a full audit log is retained for compliance review.
